Please see attached for a fact sheet on the cancer data exchange health IT standards in the US Department of Health and Human Services Health Data, Technology, and Interoperability: Patient Engagement, Information Sharing, and Public Health Interoperability (HTI-2) proposed rule. These data exchange standards, if approved, will go into effect January 1, 2028 for health IT certification.
In short, the standards cover the following:
- For ambulatory physician reporting to central registries, certified health IT must use either the most recent HL7® CDA Implementation Guide (HL7® IG for CDA© Release 2: Reporting to Public Health Cancer Registries from Ambulatory Healthcare Providers, Release 1, DSTU Release 1.1, October 2017) or the newly developed HL7 FHIR Implementation Guide (HL7® FHIR Central Cancer Registry Reporting Content IG 1.0.0-STU1).
- For cancer pathology reporting to central registries, certified health IT must use the HL7® FHIR Cancer Pathology Data Sharing Implementation Guide.
In both cases, central cancer registries would need to be able to demonstrate that they can receive and process data transmitted using these standards.
A couple of suggestions have been offered for public comment:
- Testing and implementation of these data exchange standards will require resources for central registries.
- Pathology laboratories have largely implemented the NAACCR Pathology Reporting Guidelines; changing to a FHIR exchange standard would be very challenging for the pathology laboratories and the central registries. Several groups are recommending encouraging use of the NAACCR Volume 5 standard instead. Note: this would require that NAACCR ballot and public its standard via HL7.
